Case Management
OCCK Inc. provides case management services to children and adults with developmental disabilities. We will work with you to find out what resources are available to benefit you.
Service Coordination Specialists (SCS) provide support to individuals, their guardians and families to promote independence and inclusion in their communities. We work with you and people important to you to assist with the services you need to succeed. As your partner and advocate, we will find resources, explain your rights, and help you understand systems. We are with you every step of the way!
Our case managers will be your resource for information, your advocate, and your assistant through systems while helping you plan for your future and find the services you need. We also provide 24-hour on-call services to assist you with crisis action planning. We believe our values are unique because we believe people can learn, grow, have control over their lives and will gain the skills they need to be independent. We link you to information and resources that lead you toward independence and dignity in your life.
