Independent Connection
Home and Community Based Services (HCBS)
People with disabilities are able to live independently in their communities with proper supports. Activities of daily living are often taken for granted. Examples are getting out of bed, bathing, dressing, toileting, and housekeeping.
If you need any support, Independent Connection provides a program called Home and Community Based Services. This program serves anyone who is 16-64 and Medicaid eligible. Our Case managers can assist you with the program.
