Independent Connection

Home and Community Based Services (HCBS)

People with disabilities are able to live independently in their communities with proper supports. Activities of daily living are often taken for granted. Examples are getting out of bed, bathing, dressing, toileting, and housekeeping.

If you need any support, Independent Connection provides a program called Home and Community Based Services. This program serves anyone who is 16-64 and Medicaid eligible. Our Case managers can assist you with the program.

Physical Disability Waiver

  • Do you need assistance with taking a bath and cooking meals?
  • Do you need help with making modifications to your home?
  • Do you know who to call if you need to apply for disability benefits?

Technology Assisted Waiver

  • Does your child require nursing care?
  • Is your child dependent on medical technology?

Traumatic Brain Injury Waiver

  • Have you recently sustained a head injury and need to know what services are available?
  • Do you know what services are available to assist you with cooking and house cleaning?
  • Do you need help with accessing services such as physical therapy?

Developmental Disability Waiver

  • Do you know what options are available to your child when they graduate from high school?
  • Do you need assistance with developing skills to live independently?
  • Do you want to work but need assistance in developing the skills you need to get a job?
  • Would you like someone to assist you with shopping, attending community events, or going to church?

Frail Elderly Waiver

  • Do you need information on options available other than a nursing home?
  • Do you feel safe in your home or need assistance to stay in your own home?
  • Do you need help with household tasks?

  • Has your child been diagnosed with Autism or one of its spectrum disorders?
  • Would it be helpful for your child to receive therapy or supports in your home?
